Applications are invited for an exciting new post within the Medicine and Emergency Care Division.

We looking for an enthusiastic and innovative person to provide targeted support to young people aged 16-25 affected by diabetes. You will guide them to have an increasing role in managing their own care as they transition through to adult service and sign posting to other agencies as required.

This post will play a key role in the development of the diabetes transition service, particularly looking to see how young people’s engagement with the service can be improved and help to develop a peer group network of support.

The post holder will be working within a large, experienced multi-disciplinary team across boundaries between paediatric and adult services and as such you will be exposed to all kinds of new challenges and responsibilities, to ensure you continue learning and developing as you go.
